INFORMATION TO BE INCLUDED ON UNDERGROUND PERMIT
-Landowner should be listed as Applicant and should sign the application form.
-A plat of the area (ASC aerial photo, subdivision plat, soil map, etc.) showing location of work with dimensions to property line, section corner, building site or other landmark.
-Road crossing should be flagged at the R.O.W. Line on each side of the road. Crossing shall be perpendicular to road.
Description of work should include:
1. Type or use of line (Field tile, Water, Sewer, Etc.)
2. Size and material type of line.
3. Depth of crossing from raod surface.
4. Method of Construction i.e. bore, backhoe, trencher (include width), etc.
5. Contractor's name and written estimat for road crossing.
-Applications are reviewed at Board meetings (normally Tuesday) and must be approved before work is done. Applicant shall be sent an approved copy.
-Signs for traffic control are available for use. Make arrangements for pick up and return.
-Faxed copy of permit and necessary paperwork will be accepted but originals must be submitted before work is done.
